Patients are admitted to the hospital prior to total pancreatectomy with islet auto-transplantation (TPIAT) to ensure readiness for surgery. The following lists the steps involved in the admission, organized by phase of care.
Prior to admission
Pancreas Care Center nurse practitioner will enter all admission orders
“TPIAT pre-op Total Pancreatectomy with Islet Auto-transplantation”
Pancreas Care Center surgeon will enter all intra-op orders
“TPIAT intra-op Total Pancreatectomy with Islet Auto-transplantation”
Day of admission
Admitting floor will release all admission orders, pre-op care includes:
- Place PIV
- Labs as ordered, include urine pregnancy test if applicable
- EKG (call EKG tech Mon-Fri, Surgery resident Sat-Sun)
- Incentive spirometer teaching
- Bowel prep with Miralax 2 caps three times daily
- Home medications as ordered
- CHG bath/shower evening prior to OR
- Clear liquid diet until midnight
- NPO with IVF (D5 ½ NS 10 mEq/L at maintenance rate) at midnight
PICU educator (or designee) will release ALL intra-op orders on the day of admission AFTER the admission orders are active. This allows sufficient time for pharmacy to prepare all of the medications, antibiotics and drips required for the surgery. Medications will be dispensed to the OR/PICU for the case.
- Admitting floor should not do anything with the intra-op orders
Day of surgery
Admitting floor to do the following on the day of surgery:
- Administer Lovenox as ordered at 0530 the morning of OR
- Send on call antibiotics to the OR with patient (fluconazole, Meropenem &
Vancomycin). Note Meropenem may be stored in the med refrigerator.
Pancreas Care Center nurse practitioner will enter all post op orders
“Post op TPIAT Total Pancreatectomy with Islet Auto-transplantation” and “TPIAT Post op Labs First 48 Hours Total Pancreatectomy with Islet Auto-transplantation”
