Operative repair in congenital diaphragmatic hernia: How long do we really need to wait?

Study of 158 CDH neonates demonstrates that oxygenation index ≤9.4 predicts survival and represents physiologic stability for surgical repair. Delaying repair beyond this threshold increases ventilator days and hospital length of stay without improving survival, suggesting earlier intervention may optimize outcomes in stable patients not requiring ECMO.
