Intestinal malrotation in extremely premature infants: a potential trap

Retrospective study of 514 extremely premature infants (<28 weeks) found 1.4% prevalence of symptomatic intestinal malrotation, seven times higher than term infants. Unlike typical presentation, these patients showed marked abdominal distension without bilious vomiting, often with subacute non-specific symptoms, requiring high clinical suspicion for timely surgical intervention.
