Multicenter analysis of 1,783 pediatric ovarian torsion cases reveals 22.5% underwent oophorectomy, with higher rates in younger children, those with public insurance, and complex chronic conditions. Findings highlight significant practice variation and call for standardized ovarian preservation protocols to reduce unnecessary organ loss.
