Histologic analysis of 15 pediatric short bowel syndrome patients undergoing initial and repeat STEP procedures reveals persistent mucosal inflammation in most cases, with comparable villus morphology and enterocyte dynamics between surgeries. Younger age and reduced crypt apoptosis at initial STEP may predict need for repeat intervention.
