Response to "Fat-anchoring orchidopexy": Some concerns

We read with great interest the article on fat anchoring orchidopexy by Khaled, et al. [1]. It is essentially a technique of subdartos orchidopexy, which is currently the standard of care. The authors merely introduced a modification of routing the testis to the subdartos pouch through a fat pad. We wonder as to where from the fat pad mentioned by authors this is derived. The scrotum is generally devoid of fat. The presence of fat, which is a good thermal insulator, will prevent heat dissipation from the scrotum, thereby defeating the very purpose of testicular descent.
