Comment on: “Management of simple appendicitis in children: Early appendectomy vs. non-operative management with subsequent appendectomy for failure: Does initial approach impact surgical complications?”

The retrospective cohort study by Becker et al. presents an insightful comparison of early appendectomy versus appendectomy after failed non-operative management (NOM) in children with simple acute appendicitis [1]. However, one critical area deserves further exploration the unexpectedly high proportion (22.2 %) of histological normal appendices in the subsequent appendectomy group. This raises concerns regarding the diagnostic accuracy of recurrent appendicitis, which forms the basis for delayed surgery post-NOM.
