Objetivo: evaluar el volumen testicular en la pubertad en niños que se sometieron a orquidopexia a los 9 o 36 meses en comparación con niños con descenso posnatal espontáneo.
Métodos: a los 6 meses de edad, los niños con criptorquidia unilateral congénita fueron asignados aleatoriamente a cirugía a los 9 o 39 meses de edad y se les dio seguimiento hasta los 16 años en paralelo con los niños con descenso postnatal espontáneo. Se realizó ecografía a los 11 y 16 años para determinar volumen testicular. La proporción entre el testículo inicialmente no descendido y su contraparte escrotal se utilizó para evaluar el crecimiento testicular.
Resultados: A los 16 años, la proporción fue menor (p < 0,00) en el grupo tardío en comparación con el grupo temprano. A los 16 años, los testículos descendidos espontáneamente eran significativamente más pequeños que sus homólogos escrotales pero más grandes que los grupos operados (p <0,01 temprano y p <0,00 tardío).
Conclusión: Nuestros datos a los 16 años muestran que la orquidopexia a los 9 meses da como resultado un mejor crecimiento testicular en comparación con los 3 años, pero no alcanzó los volúmenes correspondientes de sus contrapartes escrotales. Esto indica que una cirugía más temprana es beneficiosa para el crecimiento testicular. A los 16 años, los testículos descendidos posnatalmente no sólo eran más grandes que los testículos tratados quirúrgicamente sino que también presentaban un crecimiento testicular deficiente.
