Abstract

Introducción

Los pacientes adultos con colecistitis que no se someten a colecistectomía en el ingreso inicial tienen peores resultados; sin embargo, hay escasez de datos sobre el papel de la colecistectomía durante la hospitalización inicial en la población pediátrica. Nuestro objetivo fue determinar los resultados y las tasas de reingreso entre los pacientes pediátricos con colecistitis que se sometieron a una colecistectomía inicial frente a los que no.

Métodos

Realizamos un estudio retrospectivo de niños (<18 años) ingresados ​​con colecistitis aguda (CA) que requirieron hospitalización utilizando la Base de datos nacional de readmisión (NRD) de 2018. Los criterios de exclusión incluyeron edad ≥ 18 años y muerte en el momento del ingreso inicial. Se realizó un análisis de regresión logística multivariable para identificar factores asociados con los reingresos a los 30 días.

Resultados

Identificamos 550 ingresos por colecistitis aguda de índice único. La edad media fue 14,6 ± 3,0 años. La mayoría de los pacientes eran mujeres (n = 372, 67,6%). La colecistectomía índice se realizó en (n = 435, 79,1%) de los casos. La tasa de reingreso a los treinta días fue del 2,8% en los pacientes que se sometieron a colecistectomía índice y del 22,6% en los que no (p <0,001). En el análisis multivariable, los pacientes que no se sometieron a colecistectomía índice tuvieron mayores probabilidades de reingreso a los 30 días que aquellos que no se sometieron (OR 10,66; IC del 95 %: 5,06 a 22,45, p < 0,001). Las pacientes femeninas también tenían mayores probabilidades de reingreso a los 30 días en comparación con los hombres (OR 3,37; IC del 95 %: 1,31 a 8,69).

Conclusiones

Los pacientes que no se sometieron a una colecistectomía índice tuvieron un aumento de más de diez veces en las probabilidades de reingreso a los 30 días. Se requiere más investigación para comprender las barreras para la colecistectomía inicial a pesar de las recomendaciones de la sociedad y el claro beneficio clínico.
