Intraoperative Technical Considerations
No trans-anastomotic tube should be placed prior to completion of the post-operative esophagram. Prosthetic interposition material must not be used between the esophageal and tracheal suture lines to minimize complications and optimize healing.
Postoperative Antibiotic Management
Discontinue prophylactic antibiotics used for surgical site infection prevention within 24 hours of surgery. Extended antibiotic courses are not indicated in uncomplicated cases.
Postoperative Imaging Protocol
Obtain contrast esophagram on postoperative day 5, or the next business day if POD5 falls on a weekend or holiday. This study evaluates anastomotic integrity and identifies potential leaks or strictures before initiating oral feeding.
Evidence Base and Consortium Data
This protocol is derived from outcomes data published by the Midwest Pediatric Surgery Consortium challenging traditional surgical dogma in proximal esophageal atresia with distal TEF management. The evidence supports simplified perioperative care without compromising patient safety.
