The native esophagus is the optimal conduit for esophageal atresia (EA) repair and esophageal preservation is a primary tenet of surgical management [1–4]. In long-gap EA, esophageal reconstruction with delayed primary anastomosis has shown superior long-term functional outcomes when compared to esophageal replacement [2–4], and the goal of achieving delayed repair of the native esophagus remains a well-established priority [1-3,5]. However, safely achieving a primary handsewn anastomosis in long-gap EA can often be challenging.
