This letter discusses emergency management of tension gastrothorax, a life-threatening complication of congenital diaphragmatic hernia, in a 3-year-old patient who required intubation and transfer after acute deterioration. The case highlights the importance of rapid recognition and intervention in pediatric gastrothorax presentations.

  • Chest x-ray can be diagnostic for tension gastrothorax in pediatric patients presenting with respiratory distress and vomiting.
  • Thoracic needle decompression may serve as a temporizing measure to stabilize critically ill children with tension gastrothorax.
  • Acute deterioration in suspected tension gastrothorax warrants immediate airway management and transfer to specialized pediatric surgical care.
