In brief
This case report describes a 6-week-old infant with congenital lobar emphysema in the right upper lobe, diagnosed after prolonged respiratory symptoms following viral infection. The surgical challenge centered on intraoperative identification of the affected lobe due to minimal gross pathological changes compared to healthy lung tissue.
- Congenital lobar emphysema can present after viral infection in young infants, complicating diagnosis.
- CT imaging is essential for confirming congenital lobar emphysema when clinical presentation is atypical.
- Intraoperative identification of emphysematous lobes can be challenging due to minimal gross pathological changes.
- Lobectomy remains definitive treatment despite surgical difficulty in distinguishing affected from healthy tissue.
- Early postoperative recovery is typical following successful resection of congenital lobar emphysema.

An unusual severe and prolonged respiratory ailment following a common viral infection in a young infant of 6 weeks prompted us for a chest computed tomography that detected a right upper zone congenital lobar emphysema. The infant underwent a lobectomy (Fig. 1 and supplementary figure) and was discharged soon from the intensive care unit. Compared to the healthy lungs, the affected emphysematous lobe has minimal gross appearance of damage. This led to a surgical dilemma arising in this young infant, when the clarity between the emphysematous and healthy pulmonary lobes was less clear.
