In brief

In brief

This study compares two surgical techniques for identifying lung segment boundaries during segmentectomy in infants with congenital pulmonary airway malformation. The authors evaluate ICG fluorescence imaging against traditional inflation-deflation methods, addressing a critical technical challenge in pediatric thoracic surgery where anatomical variations complicate precise resection.

  • ICG-guided NIRF imaging offers precise intersegmental plane identification in infant lung segmentectomy for CPAM despite complex anatomy.
  • Modified inflation-deflation method remains a viable alternative when fluorescence imaging is unavailable in pediatric thoracic surgery.
  • Accurate ISP identification is critical in infant segmentectomy to preserve healthy lung tissue while completely resecting malformations.
  • Comparative data on ISP identification techniques in infants is scarce; this study addresses a significant gap in pediatric surgical literature.
  • Both techniques show distinct perioperative profiles; surgeon familiarity and institutional resources guide method selection.

The identification of the intersegmental plane (ISP) is a crucial step in segmentectomy for children with congenital pulmonary airway malformation (CPAM) due to complex anatomical variations. However, there is very limited literature available on this aspect specifically for infant. In this study, we compared the intravenous indocyanine green (ICG)-guided near-infrared fluorescence (NIRF) imaging method with the modified inflation-deflation method in terms of their perioperative characteristics and summarized our experience.
