In brief

This article examines the reliability of ultrasound in detecting complicated appendicitis in pediatric patients, a critical distinction that influences treatment decisions. Accurate preoperative identification helps guide whether surgical intervention is needed or if antibiotic therapy alone may suffice for uncomplicated cases.

  • Preoperative identification of complicated appendicitis guides treatment strategy and may avoid unnecessary surgery.
  • Delayed diagnosis of acute appendicitis increases risk of complications, longer hospital stays, and postoperative morbidity.
  • Growing evidence supports antibiotic-only treatment for uncomplicated acute appendicitis in pediatric patients.

While acute appendicitis (AA) is one of the most frequent emergency surgical conditions, diagnosing it continues to be challenging [1–3]. This can lead to late diagnosis of AA with complications, longer hospital stays, and higher postoperative morbidity [2]. Preoperative identification of children with complicated appendicitis (CA) is crucial in guiding treatment strategy, not least in the light of growing evidence supporting the use of antibiotics alone in the treatment of uncomplicated AA [4].
