In brief

In brief

This commentary discusses novel detrusor pressure curve indices (Simpson and Trapezoidal) that outperform traditional leak point pressure measurements in predicting complications of pediatric neurogenic bladder, including upper tract dilation, vesicoureteral reflux, and recurrent UTIs. The approach offers a more comprehensive urodynamic assessment than single-point pressure measurements.

  • Simpson and Trapezoidal indices outperform detrusor leak point pressure in predicting upper tract dilation and VUR in pediatric NLUTD.
  • Detrusor pressure curve area indices provide continuous assessment, overcoming DLPP's limitation as a single-point measurement.
  • SI/TI show promise as superior urodynamic markers for recurrent UTI risk stratification in children with neurogenic bladder.
  • Curve-based indices may improve clinical decision-making for timing of interventions in pediatric neurogenic bladder management.

We read with interest Costa–Roig et al.‘s study on detrusor pressure curve area indices (Simpson [SI], Trapezoidal [TI]) in pediatric neurogenic lower urinary tract dysfunction (NLUTD) [1]. Their demonstration that SI/TI surpass detrusor leak point pressure (DLPP) in predicting upper tract dilation (UTD), vesicoureteral reflux (VUR), and recurrent urinary tract infection (Tables 1–4) provides a clinically actionable solution to DLPP's limitation as a snapshot parameter [2,3].
