Young children with perforated appendicitis benefit from prompt appendectomy

Prospective study of 176 children with perforated appendicitis found that nonoperative management failed in 32% of cases, with younger age and early presentation (<2.75 days from symptom onset) independently predicting failure. Immediate appendectomy resulted in better outcomes than nonoperative treatment, particularly in young children who perforate rapidly.
