Primary vs. Salvage Liver Transplantation for Biliary Atresia: A Retrospective Cohort Study

This SRTR/OPTN database study of 3,438 children with biliary atresia found that Kasai hepatoportoenterostomy remains appropriate first-line treatment, as it delays transplant beyond infancy in most patients without compromising subsequent transplant outcomes. Primary liver transplantation offers no survival advantage over salvage transplantation, even after early Kasai failure.
