Making the diagnosis of midgut volvulus: Limited abdominal ultrasound has changed our clinical practice

Retrospective study of 195 cases demonstrates that limited abdominal ultrasound achieves 92% diagnostic rate with 100% positive and negative predictive values for midgut volvulus when diagnostic, using SMA cutoff and whirlpool signs. This high accuracy supports ultrasound as first-line imaging, reserving upper GI studies only for nondiagnostic cases to reduce radiation exposure in pediatric patients.
