Heterotopic pancreas (HP) is a congenital anomaly defined as the presence of pancreatic tissue outside its normal location that lacks anatomic or vascular continuity with the pancreas itself. It is often asymptomatic, but can present with pain, gastric outlet obstruction, vomiting, and dysphagia. Pyloric obstructions from HP can cause several of these symptoms and require surgical intervention. This case report documents the first surgical resection of a symptomatic HP using a laparoscopic transgastric approach, under endoscopic guidance preformed on an 18-year-old male. This innovative method is a minimally invasive option for HP resection compared to open abdominal surgery, allowing for a faster recovery and limited complications.
