Propósito

Los catéteres venosos centrales tunelizados (TCV) se usan comúnmente para la quimioterapia pediátrica. Recientemente, en su lugar se han utilizado catéteres centrales de inserción periférica (CCIP). Aunque el PICC tiene las ventajas de una inserción más sencilla y menos complicaciones graves, hay poca información sobre la eficacia del PICC en comparación con la TCV en la quimioterapia pediátrica.

Métodos

Se incluyeron en el estudio pacientes menores de 18 años con neoplasia maligna primaria que recibieron quimioterapia con PICC o TCV en nuestra institución desde diciembre de 2007 hasta agosto de 2022. Comparamos retrospectivamente PICC y TCV utilizando registros médicos.

Resultados

Durante el período de observación, se insertaron 133 catéteres (73 PICC y 60 TCV). La mediana del tiempo de permanencia fue de 99 días para los PICC y de 182 días para los TCV, siendo los TCV significativamente más largos (p < 0,001). No hubo diferencias significativas en la incidencia de complicaciones, como infecciones, trombosis, obstrucción o accidentes mecánicos. Al comparar los pacientes tratados con PICC (grupo PICC) versus aquellos con TCV (grupo TCV), el tiempo desde el diagnóstico hasta la inserción fue significativamente más corto en el grupo PICC (p <0,001). En el grupo PICC, ninguno de los pacientes requirió anestesia general y la quimioterapia se completó con PICC únicamente.

Conclusión 

PICC puede ser una alternativa a TCV en quimioterapia pediátrica.
