In brief
In brief
This study demonstrates that machine learning algorithms analyzing free-text clinical notes can identify pediatric epilepsy surgery candidates as accurately as physicians, with F-measures of 0.71-0.82, and can do so up to one year earlier than documented referrals. The research systematically evaluates factors affecting classifier performance and shows potential for reducing delays in surgical intervention for drug-resistant epilepsy.

Statements in this article
Machine learning methods can identify epilepsy surgery candidates as well as physicians do.
Machine learning methods can identify candidates earlier than physicians do.
F-measures for identifying epilepsy surgery candidates using machine learning ranged from 0.71 to 0.82.
Feature set, classification algorithm, amount of training data, class balance, and gold standard all significantly affected classification performance.
Classification performance was better than the highest agreement between two annotators, even at one year before documented surgery referral.
Machine learning methods can contribute to reducing lag time to surgery referral for pediatric epilepsy patients.
