Meta-analysis of 1288 pediatric patients demonstrates that treatment strategy for complicated appendicitis should differ by presentation type. Children with appendicular abscess/phlegmon benefit from initial non-operative management with lower complications and readmissions, while those with free perforation achieve better outcomes with early appendectomy.
