In brief
This letter responds to a study evaluating near-infrared spectroscopy (NIRS) for diagnosing testicular torsion in children with acute scrotal pain. The authors agree with the original conclusion that NIRS is not clinically suitable for differentiating testicular torsion and emphasize the need to reinforce this finding in pediatric urology practice.
- Trans-scrotal NIRS is not clinically reliable for diagnosing testicular torsion in pediatric patients.
- Clinical examination and ultrasound remain the gold standard for evaluating acute scrotal pain in children.
- Novel diagnostic technologies require rigorous validation before replacing established diagnostic pathways.

We read with great interest the recent article “Testicular pain in children: Near-infrared spectroscopy cannot differentially diagnose underlying” by Carolina et al. evaluating the role of trans-scrotal near-infrared spectroscopy (NIRS) in children with acute scrotal pain [1]. The conclusion of this article is that NIRS is not suitable to diagnose testicular torsion (TT) in children in clinical practice. We fully agree with this conclusion, yet we believe it deserves further reinforcement.
