In brief
In brief
This study compares two lipid emulsions used in neonatal parenteral nutrition—soybean oil-based Intralipid and fish oil-based Omegaven—focusing on their potential to displace bilirubin from albumin. Understanding this displacement mechanism is critical for preventing bilirubin-induced neurologic dysfunction in vulnerable neonates receiving intravenous nutrition.
- Intralipid increases unbound free fatty acids, which displace bilirubin from albumin and raise BIND risk in neonates.
- Omegaven (fish oil-based lipid) offers hepatoprotective benefits but its bilirubin displacement profile requires further study.
- Clinicians should monitor bilirubin levels closely when using lipid emulsions in jaundiced neonates on parenteral nutrition.

Intralipid, a soybean oil-based fat emulsion, is the only FDA-approved lipid source for neonatal parenteral nutrition, but it increases unbound free fatty acids (FFAu), which can displace bilirubin from albumin and elevate the risk of bilirubin-induced neurologic dysfunction (BIND). Omegaven, a fish oil-based alternative, exhibits hepatoprotective properties; however, its effect on bilirubin displacement is less well understood.
